Hourly Consultations are a great option for those who would like to meet me to discuss their needs, or the needs of a family member and to see if I am a ”good fit”.
They are also ideal for taking the services available as packages, but billed on an hourly basis (as required by some extended benefits plans).
Step-by-Step instructions for Booking Your Appointment:
As easy as 1-2-3
- Download the Intake and Service Option Form to your desktop, then fill it out on your computer (not by hand, please!)
- Email the completed pdf form*, any available recent blood test results and your availability to the email address on the form
- Pay for your services below via credit card (includes $10.00 credit card charge plus GST), or you can send an e-transfer and save the credit card charges.
*If you would like a password-protected form, please send me a note using the Contact Me form on the tab above.
* NOTE: If you don’t already have it, you can download the free Adobe Acrobat Reader here to complete the pdf form on a Windows-based computer. If you are on another type of computer, a phone or a tablet, then you can use Adobe Acrobat online fill and sign tools.
