Cash-flow can be challenging. The Flexible Payment Option will enable you to pay for each of the three components of services in the Complete Assessment Package one-by-one.
Step-by-Step instructions for Booking Your Appointment:
As easy as 1-2-3
- Download the Intake and Service Option Form and complete it on your computer (not by hand, please!) and also download and complete the Flexible Payment Option Form
- Password-protect the forms for confidentiality*, then email the completed forms and any available recent blood test results and your availability to the email address on the form. Please send the password to the form in a separate email.
- Pay for your services below via credit card (includes $10.00 credit card charge plus GST), or you can send an e-transfer and save the credit card charges
*If you would like a password-protected form, please send me a note using the Contact Me form on the tab above.
