The Complete Assessment Package* is an all-inclusive package for clients in Canada who want to eat healthier and/or lose (or gain) weight, or are seeking to put the symptoms of metabolic disease (type 2 diabetes, high blood pressure, high cholesterol) into remission.
This package provides a total of 3 hours of services.

Step-by-Step instructions for Booking Your Appointment:
As easy as 1-2-3
- Download the Intake and Service Option Form to your desktop, then fill it out on your computer (not by hand, please!)
- Email the completed pdf form* and your most recent complete blood test results, along with your availability to the email address on the form
- Pay for your services below via credit card (includes $15.00 credit card charge plus GST), or you can send an e-transfer and save the credit card charges

* NOTE: If you don’t already have it, you can download the free Adobe Acrobat Reader here to complete the pdf form on a Windows-based computer. If you are on another type of computer, a phone or a tablet, then you can use Adobe Acrobat online fill and sign tools.
